    Patient Guidance System

    Hammeras Group created a patient guidance system designed to work alongside Cancer Teamwork Response and the broader CTR Network.

    Developed and led by Candy Hammeras, the system brings together healthcare relationships, practical experience, and non-clinical guidance to help patients and families move more effectively through complex points in their care.

    Through this connected model, Hammeras Group works with healthcare professionals, specialists, medical centers, and community partners, while CTR provides patients and families with direct guidance at no cost.

    The system supports both adults and children, helping patients prepare for important conversations, understand what may require clarification, and connect with appropriate medical expertise when needed.

    Operating independently from any single medical facility allows Hammeras Group and CTR to collaborate across institutions, guided by the needs of the patient and in support of the healthcare professionals involved.

    Through decades of experience in hospital administration, patient relations, and physician relations, Hammeras Group has observed healthcare from multiple perspectives.

    The organization understands the pressures facing physicians, hospital teams, and care providers. It has also seen how quickly patients and families can become overwhelmed by unfamiliar medical information, complex decisions, multiple offices, and unclear next steps.

    While the quality of medical care is often excellent, the path between points of care is not always connected.

    That gap became a central focus of Hammeras Group’s work.

    Drawing on extensive healthcare experience, professional relationships, and a deep understanding of how healthcare systems operate, Hammeras Group began helping patients prepare for important conversations, connect with appropriate expertise, and move more effectively through the care process.

    That work evolved into the patient guidance system now shared across Hammeras Group, Cancer Teamwork Response, and the CTR Network.

    It is built on experience and a simple belief: patients should not have to navigate complex healthcare decisions alone.

How The CTR Network Works

The CTR Network grew from Hammeras Group’s long-standing relationships with physicians, specialists, medical centers, and community leaders across Southern California.

It expands those relationships into a broader healthcare ecosystem connecting the professionals who deliver care, the communities that support patients, and the donors who make no-cost guidance possible.

Within the network, healthcare professionals can strengthen referral pathways, share trusted resources, and collaborate when a patient’s needs extend across specialties or organizations. Hammeras Group serves as the professional bridge, while Cancer Teamwork Response provides direct guidance to patients and families.

Together, the CTR Network creates a more connected approach to healthcare, with distinct participants working toward one shared priority: helping patients reach the right support at the right time.

Navigating Our Healthcare System Since 2012

From Experience to a Connected System

Candy Hammeras spent more than two decades in hospital administration, patient relations, and physician relations. Through that work, she saw both the dedication of healthcare professionals and the challenges patients and families faced when trying to move through a complex and often fragmented system.

She recognized that even when excellent medical care was available, patients could still struggle with unclear next steps, disconnected communication, limited access to specialists, and the administrative burden surrounding a serious diagnosis.

Candy founded Hammeras Group to address those gaps through patient-relations strategy, healthcare collaboration, and non-clinical guidance. As more individuals and families sought her help with rare and complex diagnoses—many involving cancer—the work evolved into a broader patient guidance system.

That experience ultimately led to the creation of Cancer Teamwork Response, ensuring that patients and families could receive guidance at no cost, regardless of their ability to pay.

Today, Hammeras Group, CTR, and the CTR Network work together as one connected ecosystem—bringing healthcare professionals, community partners, donors, and patient guidance into closer alignment, with the patient always at the center.
